Passports classed as wear and tear can be accepted as evidence of nationality and identity.

How do I replace a lost mutilated passport?

To replace a lost passport, you will need to apply in person and include the following:

  1. Form DS-64 (Statement regarding lost or stolen passport)
  2. Form DS-11 (Application for U.S. passport)
  3. Citizenship evidence* (e.g. birth or naturalization certificate)
  4. A photocopy of citizenship evidence.
  5. Present ID (in person)

If your previous passport was lost or stolen, you'll have to apply for a new one in person. You'll need to bring two forms in this case: the standard DS-11 passport application and Form DS-64, which asks you to describe what happened to your little blue book.
